Several of OTD’s forms are available only as PDF files. These forms require an original signature. You may download and print the form, but must return a signed original copy.

PDFs are marked with an * to indicate that the file requires the Adobe PDF reader. This program is available free from Adobe. Click on the icon below. When you get to the Adobe page, select ‘Downloads’ then ‘Free Acrobat Reader’.
